By signing below, the applicant represents and warrants to the Foundation that (i) it has read and understands the Scholarship Guidelines and (2) all information provided by the applicant to the Foundation in this application form, any supplementary documentation or otherwise is true and correct as of the date of submission. To the extent any information provided by the applicant becomes untrue or inaccurate after the date submitted, the applicant agrees to immediately notify the Foundation at scholarships@farrellboy.org. and provide any updated or supplementary information or materials requested by the Foundation. If a minor is the subject of this application form, I hereby warrant that I am the parent or legal guardian of the minor, that I am at least 18 years old, and that I have the right to contract in the minor’s name.

For the purpose of encouraging promotion of philanthropy and charitable values, and for other good and valuable consideration, and to the extent that I am selected to receive any award from the Foundation (as defined below), I hereby grant to Farrell Boy Foundation, Inc. (and those acting with its permission and authority) and their respective board members, officers, employees, agents, successors and assigns (the “Foundation”), the right to use and publish my name, information (as it relates to receiving any award by the Foundation) and photographic images and pictures of me or in which I am included on its website or in its promotional materials for the sole purpose of promoting the Foundation and its charitable purpose. To the extent that the Foundation’s selection committee is required to collaborate with an educational institution in selecting award recipients, I hereby grant the Foundation the right to share the information I have provided to them in my application for an award, on a confidential basis, with such educational institution for such purpose. I agree to release, indemnify, defend, and hold the Foundation harmless from any and all actions, claims, costs, and expenses, including, without limitation, any claims for defamation, false light or invasion of privacy, resulting from use of my name, image, and likeness.
